Guided Reading
	Good 4 pts	Fair 3 pts	Poor 2 pts	Unacceptible 1 pts
On Task	Good Student reads the entire selection in the time allotted.	Fair Student reads in the time allotted but does not finished due to minimal distractions.	Poor Student is distracted and does not complete reading task.	Unacceptible Student refuses to read.
Fluency	Good Student is always making sense of the text as opposed to simply decoding words. Meaningful reading instead of word by word reading.	Fair Student attempts to read selection but may have trouble due to breaks in fluency. Some short phrases with some word by word reading.	Poor Student reads word by word.	Unacceptible Student refuses to read out loud.
Use of Strategies for Understanding	Good Student uses self-help strategies to understand what is being read. (re-reads passage, stops and goes back, looks up information)	Fair Student seeks occasional teacher assistance to understand what is being read.	Poor Student seeks frequent teacher assistance to understand what is being read.	Unacceptible Student refuses to read without teacher assistance.
Understands Elements	Good Student understands text elements without teacher prompting.	Fair Student knows some elements without prompting.	Poor Student needs prompting or hints to assist with elements.	Unacceptible Student refuses to answer prompts about elements.
Key Facts	Good Student can identify a minimum of 5 facts without promtping.	Fair Student can identify 3-4 facts from reading.	Poor Student can identify 1-2 facts from reading.	Unacceptible Student cannot or refuses to identify key facts.
Prior Knowledge	Good Understands the goal for reading and taps into the prior knowledge needed. Makes connections, predictions and/or draws inferences.	Fair Requires some prompts from teacher to tap into prior knowledge. May offer some voluntarily and require prompts for others.	Poor Requires prompts to tap into prior knowledge.	Unacceptible Student does not participate in prior knowledge.
