You continuously distract peers while they are working. You leave your seat without permission and disturb the overall learning atmosphere in classroom. (5 pts)
Fair Behavior

You occasionally distract peers, but stop after being spoken to by the teacher. You stay in your seat. (10 pts)
Excellent Behavior

You stay in your seat until given permission to move. You complete assignments quietly and with effort. You raise your hand when needing teacher assistance. (25 pts)
Interaction with Staff

Poor Behavior

You are disrespectful when speaking (responding) to teacher/staff. (5 pts)
Fair Behavior

You sometimes ignore teacher/staff when they speak to you, but you do not talk back or use profanity. (10 pts)
Excellent Behavior

You respond appropriately when spoken to by teacher/staff. (25 pts)
Staying on Task

Poor Behavior

You are constantly a distraction to others. You continuously finds ways to avoid the tasks (ie;going to the bathroom constantly, getting a drink). Assignments are done with little or no effort (5 pts)
Fair Behavior

You are on task 75% of the time. Finish most assignments with fair effort. (10 pts)
Excellent Behavior

You are on task 95% of the time. All assignments are completed. Work is done with best effort. (25 pts)
Following Directions

Poor Behavior

You ignore directions given by teacher/staff. You require the teacher to tell you 3 or more times to complete your assignment. (5 pts)
Fair Behavior

You follow direction but show that you are not happy about doing it. You complete most work but with poor effort. (10 pts)
Excellent Behavior

You complete assignments with as much effort as possible the first time given. (25 pts)
