Classroom	Poor The classroom is unorganized. I can't find anything. The walls are distracting. I don't know where to turn in assignments or get supplies.	Fair The classroom is kind of organized. The walls may or may not be distracting. I can generally find what I need.	Good The classroom is a great place. The walls are decorated but do not distract. I know where to turn in assignments and find supplies.
Lessons	Poor The lessons in class are always too hard or too easy. I am always bored. I have not learned anything this year.	Fair The lessons in class are ok. They could be more organized. I sometimes have difficulty understanding what is being taught.	Good The lessons are good. They are organized, introduced well and I understand what I am supposed to learn from them.
Teacher availability	Poor My teacher is never available for help or gets distracted easily. I do not get the help I need to complete assignments. When my teacher does come, they do not help me.	Fair My teacher is available for help but sometimes forgets I need help. I do not get the help I need in class and must come in before or after class.	Good My teacher is always available to help. I know when they are coming and what to do while I am waiting. When my teacher does come, I get the help I need.
Teacher approachability	Poor I would never ask my teacher for help. She is not approachable.	Fair Sometimes I can ask my teacher for assistance. Sometimes I cannot. It depends on the day or time.	Good I can always ask my teacher for help. She is always willing to help me.
Classroom Behavior	Poor The class is always off task. My teacher has no control.	Fair The class is generally good but sometimes my teacher looses control.	Good The class is always well organized and my teacher has good control.
Volume Level	Poor The class is always way too loud.	Fair The class volume is ok. The volume does not interfere with my learning.	Good The class volume is good. It never interferes with my learning.
