Journal/Learning Log Entry	UNACCEPTABLE Journal Question has no response.	FAIR Response has 1-3 lines. There was an attempt to answer the question, but the answer is unclear, or does not directly answer the question. Student did not restate the question.	GOOD Response has at least 4-5 lines. Student restated the question and their answer makes sense.	EXCELLENT!!! Response has at least 5-8 lines. Student restated the question and answered it fully, providing at least one example to support their argument/opinion.
Planner Check	UNACCEPTABLE Planner is blank.	FAIR Only some subjects have been filled in. Information is missing.	GOOD There is something written for each subject, but some information is missing.	EXCELLENT!!! All subjects are filled in! Student wrote what they learned about in each class and what was for home-work in each class. If there was no home-work, student wrote, "No Homework."
Time Management	UNACCEPTABLE Student did not get anything accomplished during the Study Skills period. Student was continuously disruptive to others.	FAIR Student needed several reminders to stay on task. Student got some work done, but did not make full use of class time.	GOOD Student needed reminders to stay on task, but used the majority of their class time wisely.	EXCELLENT!!! Student was on task majority of the period. Student used time wisely to get assignments completed, to study for upcoming tests, and/or to silent read at their desk.

FAIR Has folders or sections in notebook, but papers are not in punched and in the correct section/folder; student is able to locate some assignments.	GOOD Most papers are punched and placed in the correct section/folder; student is able to locate most assignments.	EXCELLENT!!! All papers are punched and placed in the correct secition/folder and student is able to locate specific items quickly; student is able to locate all assignments
