I generally am on task and works productively. I get my assignments done on time and they are done neatly.

I am mostly on-task and worked productively. I try hard to get my work done on time and to do my work neatly.

I am mostly off-task and unproductive. I have trouble getting my work done on time and in an orderly and neat manner.

I am rarely on-task or productive. I am not handing work in on time and it is often messy.

I contribute to activities, questions and class discussions regularly and in a respectful manner. I am engaged in class all the time.

I respond to my teachers, EA's and other students in my class appropriately. I am appropriate in my responses.

I respond in class when asked and I am appropriate in my responses.

I rarely respond in class to questions. I may be disrespectful at times.

I do not respond in class. I limit my interactions or I am distracting others.
Listening Skills

Excellent

I listen to directions and am able to follow them. I am able to work productively on assignments without bothering other students.

I listen to directions most of the time. I am usually able to do my work without needing further instructions.
Fair

I often miss instructions and then have difficulty in doing my work. This leads to disruptions and distractions for other students.
Poor

I do not listen to instructions. I am unable to be productive in class.
Attitude

I feel as though I have a positive attitude and am a positive member of my class. I understand the classroom and school expectations and I follow them.
Good

I am usually positive and follow the expectations of the classroom and school.
Fair

I am sometimes negative and/or disrespectful. I do not always follow the expectations of the classroom and/or school.

I am often negative and disrespectful in class and in the school.
