Self Assessment
	Outstanding 3 pts	Satisfactory 2 pts	Needs Improvement 1 pts
Following Directions	Outstanding I work hard to follow directions in the classroom and special classes (library, art, P.E., etc).	Satisfactory I need directions to be repeated to me sometimes and I following them some of the time.	Needs Improvement I have trouble following directions and being on task.
Line Up Quietly	Outstanding I line up on zero and wait for instruction from the teacher.	Satisfactory I line up but usually have to be reminded to get on zero.	Needs Improvement I rarely line up on zero and usually have to try again.
Always Prepared	Outstanding I turn my homework in on-time and have my supplies ready at the begining of each lesson.	Satisfactory I sometimes turn my homework in late or forget my assignments. I usually have to borrow for supplies.	Needs Improvement I forget my assignments alot and begin getting my supplies after the teacher has already begun the lesson.
Show Respect	Outstanding I respect those around me including keeping my desk neat and saying please and thank you.	Satisfactory I sometimes act rude to others and my desk "spills" onto their space. I can distract others from learning sometimes.	Needs Improvement I often distract others from learning by talking or horseplaying. I have a messy desk area, and I am reminded to use kind words.
Hands, feet, objects	Outstanding I work hard to keep my hands, feet, and other objects to myself. I don't hurt others or horseplay in the classroom.	Satisfactory I sometimes made bad choices and horseplay.	Needs Improvement I have often been called out for grabbing, tripping, kicking or throwing objects at other students.
