Emotion regulation
	Poor 1 pts	Fair 2 pts	Good 3 pts	Excellent 4 pts
Number of episodes	Poor Challenging behavioral episodes occur 3 or more times per day.	Fair Challenging behavioral episodes occur 1-2 times per day.	Good Challenging behavioral episodes occur 2-4 times per week.	Excellent Challenging behavioral episodes occur 0-1 times per week.
Intensity of behavior	Poor Behavioral episode is severe and may include injury to self or others.	Fair Behavioral episode is considerable in intensity and may include destruction of property.	Good Behavioral episode is moderate in intensity and may include talking back to adults, or saying mean things to peers.	Excellent Behavioral episode is mild and may include pouting, whining, or shutting down.
Self-regulation	Poor Requires adult to help move student to a safe place.	Fair Requires external cues from an adult to move to a safe place or help in calming.	Good Some assistance or reminders about self-calming techniques from an adult may be needed.	Excellent Student is independent in self calming techniques.
