-I am ON-TASK by completing work, staying focused, and remaining in my seat a majority of the time.
STRONG

-I am MOSTLY on-task by completing work, staying focused, and remaining in my seat most of the time.
CAPABLE

-I am MOSTLY off-task and needing several reminders to complete work started, focus on work task, and to sit back down at my desk.
POOR

-I am NOT on-task or the work that needs to be completed. I am making poor choices in not staying in my seat and not complying to teacher requests.
PREPARED FOR TASK

I am PREPARED to work on all tasks that are given to me, remember to raise my hand for assistance when needed, and listen to instructions when given to me.

I FREQUENTLY display disruptive behavior such as: speaking out of turn, derailing conversations, and distracting my classmates. Ms. Swaim has to remind me sometimes.
POOR

I display EXCESSIVE disruptive behavior such as: speaking out of turn, derailing conversations, and distracting classmates. Ms. Swaim has to remind me often.
