Take Responsibility	Good Performs all duties of assigned. Does the assigned work without having to be reminded.	Fair Performs nearly all duties. Usually does the assigned work - rarely needs reminding.	Poor Does not perform any duties of assigned. Relies on others to do the work. Needs constant reminding.
