Neuro transfers

Explained the transfer	Poor Explained to the client but did not tell about the other surfaces.	Fair Described to the client in a disorganzied manner.	Good Described to the client that you will be working on them getting in/out of chair. Describe the various surfaces you will be going to.
positioned the w/c correct	Poor Positioned chair but after several tries.	Fair Positioned chair but left too much room or had an angle.	Good Positioned the w/c close to the surface to which you will be going but not using an angle.
locked brakes & removed leg rests	Poor Had difficulty removing and locking and forgot at first.	Fair Had little difficulty locking brakes and/or removing leg rests.	Good Brakes were locked efficiently and leg rests removed efficiently.
applied transfer belt	Poor Used the "f" (fall) word in the explanation and forgot until time to move.	Fair Placed belt on client but did not explain purpose.	Good Instructed client in the application of the belt and did this efficiently.
positioned self	Poor Position could have been better but it all worked. No safety was impacted.	Fair Positioned self after several attempts.	Good Positioned self so that you are on the weak side if necessary, to the side if necessary,to the front if necessary and feet are unobstructed by equipment.
clear directions	Poor A bit disorganized and perhaps lacking some confidence.	Fair Told the client all directions but could have been more organized.	Good Told the client what they should do, where they should put their hands and legs. Told the client how to work the w/c.
Proper body mechanics	Poor	Fair Slightly off, or only bent back when sitting them down slightly.	Good Kept back straight, bent knees, did not overextend reach, stayed close to the client. Weight for the client was put on their weak side.
Proper speed in movement	Poor Moved a bit too quick, seemed to lack total control.	Fair Moved client slowly but sat them down too quick.	Good Moved client slowly, waited once standing to get their balance. Went to their weak side if applicable.
SAFETY	Poor	Fair	Good This all needs to be fine to pass this competency. If the client would have fallen you will have to redo.
