	Proficient 4 pts	Good 3 pts	Fair 2 pts	Poor 1 pts
Table of contents/Assignment Descri	Proficient 100% of items attemptedTable of Contents is 100% complete and accurate.	Good 90-99% of Table of Contents is complete and accurate.	Fair 80-89% of Table of Contents is complete and accurate.	Poor Less than 80% of Table of Contents is complete and accurate.
Stud/Adult grade & Communication	Proficient Journal has two adult signatures and Student/Adult Grade section is totaled.	Good Journal has only one adult signature but Student/Adult Grade section is totaled.	Fair	Poor Journal has no adult signatures and Student/Adult Grade section is not totaled.
Missing Sections	Proficient No sections of the journal are missing and each section is separated by dividers.	Good No sections of the journal are missing, but the dividers are missing.	Fair One-Two sections of the journal are missing. Dividers may or may not be present.	Poor Two or more sections of the journal are missing and dividers are not present.
Organization	Proficient All assignment and/or notes are kept in a logical or numerical sequence.	Good One or two assignments and/or notes are not in a logical or numerical sequence.	Fair Three or Four assignments and/or notes are not in a logical or numerical sequence.	Poor More than four assignments and/or notes are not in a logical or numerical sequence.
Work Completion	Proficient All of the assignments look 100% completed.	Good Most of the assignments look 100% completed.	Fair Few of the assignments look 100% completed.	Poor None of the assignments look 100% completed.
Neatness	Proficient Overall journal is very neat.	Good Overall journal is in a satisfactory condition.	Fair Overall journal is in a below satisfactory condition.	Poor Overall journal is unkept and very disorganized.
