Legs loaded/Knees bent

Never bent my legs for my shot. I only used my upper body.

Sometimes bent my knees and loaded my legs for the shot, but not a lot.

Guide Hand

Never had the guide hand in the proper place, on the side of the ball. I always pushed with it on the shot and "thumbed" the ball.

Sometimes had the guide hand on the side of the ball. I still "thumbed" the ball some of the time on the shot. Most of the time time I didn't use the guide hand and left it behind on the shot.
Good
Often had the guide hand in the proper place on the ball but not always. I also "thumbed" the ball some of the time on the shot. Sometimes I left it behind and did not take it up with the shot.
Excellent
Always had the the guide hand in the proper position on the ball and NEVER "thumbed" the ball on the shot. I took the guide hand up with the shot ONLY to assist the ball, not the shot.
Eyes/Aim
Poor
Never looked at a target when I shot. I simply shot the ball.
Fair
Sometimes looked at a target when I shot the ball.
Good
Often looked at a target when I shot the ball and most of the time was consistent with where I aimed. I didn't just "shoot" the ball.
Excellent
Almost always had a target when I shot the ball. I always aimed at the same spot on the rim or backboard. I was deliberate with my shot and aimed.
Follow Through
Poor
Never held the follow through and either let my arm fall downward or had a goose neck.
Fair
Sometimes held the follow through but still had a goose neck.
Good
Often held the follow through and usually did not have a goose neck. My follow through was not always consistent.
Excellent
Almost always held the follow through and NEVER had a goose neck. I reached upward toward the basket, not out.
