	Poor 0-33% 33 pts	Fair 34-66% 66 pts	Good 67-100% 100 pts
Cooperation Enter description	Poor Student is late to class more than once a week and or has poor attendance of class. Student is chewing gum, eating or drinking water in class. Student uses profanity towards other students and or the teacher. Student throws things in class, such as paper balls, pens, etc...	Fair Student is late to class more than once every two weeks and regularly attends class.	Good Student is always prompt and regularly attends class.
Work habits Enter description	Poor Student does not listen when the teacher or others talk. Student is not on task, violates class rules banning electronics and wastes time texting.	Fair Student listens when the teacher talks and students are speaking to a question asked. Occasional off task behavior.	Good Student listens when others talk, both in groups and in class. Student incorporates or builds off of the ideas of others. Student completes all assignments.
