Preparedness/Rehearsals Did you come to class ready to work? Did you follow direction and listen?	Excellent I came to class ready to work on a consistent basis. I always arrived on time, changed clothes right away and was ready to work when it was time. I took all direction and followed all instructions.	Good Arrived on time most days. Changed clothes in a regular amount of time. I may have forgotten my dance clothes a day or two.	Needs Improvement Forgot materials such as notebook/journal and assignments more than once. Arrived late to class more than three times. Did not have or put on dance attire. Did not cooperate most of dance assignment.
Behavior/Effort How did you work with others? Did you give 100%?	Excellent I was on task nearly all of the time. I gave 100% in effort and tried my hardest. I worked well with other students.	Good I was off-task occasionally. I put forth strong effort and worked fairly hard. I worked well with other students.	Needs Improvement I was off-task a lot I didn't try very hard. I didn't work well with other students.
