Circulatory physical assessment

Lung sounds	Poor Student incorrectly identifies patient's lung sounds.	Average Student accurately describes patient's lung sounds, but is unable to fully correlate disease process with physical assessment findings.	Above Average Student accurately describes patient's lung sounds: Clear, decreased, crackles, rhonchii, rales, wheezes, and correlates these sounds with the patient's diagnosis.
Heart Sounds	Poor Student cannot identify between regular normal heart sounds and abnormal heart sounds	Average Student accurately identifies patient's heart sounds with assistance of instructor and correlates with patients diagnosis	Above Average Student accurately identifies patient's both normal and abnormal heart sounds with out assistance of instructor and able to correlated with patient diagnosis
Edema	Poor Student incorrectly identifies the level of patient edema-1+, 2+, 3+, 4+ or pitting.	Average Student accurately identifies the level of patient edema, but is unable to fully correlate this finding with the patient's disease process and medication intake.	Above Average Student accurately identifies the level of patient edema and is able to correlate these finding with the patient's disease process and medication intake.
Capillary refill/legs	Poor Student incorrectly identifies the patient's capillary refill and does not identify abnormalities in the patient's lower extremities.	Average Student correctly identifies the patient's capillary refill and any abnormal findings in the lower extremities, but cannot fully correlate these findings back the patient's disease process.	Above Average Student correctly identifies patient's circulatory status stating capillary refill and lower extremity findings and correlates these findings to the pathophysiology of the patient's disease state.
Communication	Poor Student does not explain assessment to patient or talks minimally to patient.	Average Student explains the assessment as they are completing each step.	Above Average Student explains the assessment as they are completing each step and is able to answer with confidence all patient questions in a calm and efficient manner.
