Health Triangle 
Students are required to do a self-assessment of their overall health, including their physical, mental/emotional, & social health. They will critique their triangle & note where improvements can be made to create a healthy balance. In their written essay it should include goals, plans, and reflections for the future. The visual should represent YOU, it may have words or pictures to demonstrate your health. Either present, future or both. *Min of 5 pictures in each section.

Creation of an actual triangle representing the activities currently utilized to promote personal health.

Poor

Little evidence of the Health Triangle shown in the assessment. One to two total activities illustrated. Very little or no effort. Has less than the five required pictures in the sections.
Fair

Evidence of at least two sides of the Health Triangle shown in the illustration. Some effort. Has less than the five required pictures in their sections.
Good

Student uses/labels all three sides of the Health Triangle to illustrate their health. Good effort. Includes at least five pictures in each section.
Outstanding

Exceeds expectations. Colorful and creative. Complete representation of all activities. Shows great interest and effort through work. Gives the reader true insight into the author.
Reflection and Critique

A personal written reflection is completed analyzing the balance of the triangle and adding goals and plans for improvements where needed.

Poor

Limited goals listed. The Health Triangle is not very evident. <BR>
Very little development of how to achieve the goals.<BR>
Limited reflection on relating goals to future plans and health.
Fair

Evidence of two sides of the Health Triangle seen in the goals. There is reasonable development of a plan of how to achieve the goals. There is some reflection on how the goals tie into future aspirations and health.
Good

Student addresses all three sides of the Health Triangle when setting goals. Goals are clearly defined. Plans are realistic for the goals and reflect a connection to the student's future aspirations and their health. Student does a good job explain each component of health and relating it to their life.
Outstanding

Exceeds expectations. Student is reflective and analytical in the current balance of the triangle and clearly articulates a realistic plan. Student is specific in explaining their pictures and how it relates to their current situation and/or future goals.
Grammar/Punctuation/Spelling

Proofread your self-assessment & triangle for proper grammar, punctuation, and spelling.

Poor

Numerous grammatical, punctuation, and spelling errors. Language is not clear or completely helpful in delivering the message of the assessment.
Fair

Few grammatical, punctuation, and spelling errors. Language still needs more clarity in order to deliver the message of the assessment.
Good

Rules of grammar and punctuation are followed and spelling is correct. Language is clear and precise. It enhances the delivery of the reflection and critique.
Outstanding

Excellent presentation. No mistakes. Clear, well written reflection. No spelling errors, cross-outs, etc. Shows quality work and effort.
Organization

The project should flow and be presented in a logical and informative manner. Students should also have all the necessary components (questions answered) in their essay.

Poor

Essay is difficult to understand because there is very little or no sequence of information.<BR>
Student, incorrectly uses terms and is missing three or more of the necessary information.
Fair

There is difficulty following the essay because they jump around. <BR>
Student incorrectly uses terms. Student also failed to answer two or more of the required questions.
Good

Student presents information in a logical sequence which reader can follow. Student's voice is clear and uses most words correctly.
Outstanding

Student presents information in a logical, interesting sequence which reader can follow. Student answers all parts of the essay questions and thoroughly explains each component.
