	Excellent I gave my best effort and feel pleased with my performance 5 pts	Good I got the job done, feel relatively pleased with my performance, but could improve 3 pts	Needs Improvement I didn't give my best shot, and recognize that I need to improve 1 pts
Attendance/Preparedness 5 pts Did you come to class prepared and ready to learn?	Excellent Brought all needed materials to class every day.<BR> Arrived to class on time every day. <BR> Completed ALL homework assignments.	Good Brought materials to class most days. Maybe forgot 1 item. <BR> Arrived on time most days<BR> Completed homework assignments. May have used some class time to finish.	Needs Improvement Forgot materials more than once. <BR> Arrived at class late more than on time.<BR> Did not complete homework assignments
Classwork/Homework 5 pts Did you complete your work? Does it meet the requirements?	Excellent I completed my CW and HW on time and accurately.	Good I completed my CW and HW but may not have met all the criteria.	Needs Improvement I did not complete all of my CW and HW as directed, or I handed work in late
Behavior/Effort 5 pts How did you work with others? Did you give 100%?	Excellent I was on task nearly all the time;I gave 100% in effort and tried my hardest.<BR> I worked well with others, giving and receiving constructive criticism.<BR> The teacher did not have to talk to me at all about my behavior.	Good I was off-task occassionally<BR> I put forth strong effort and worked fairly hard.<BR> I worked well with others but didn't really give helpful comments on their work.<BR> The teacher had to talk to me about my behavior once in awhile	Needs Improvement I was off-task a lot. I didn't try very hard. I didn't work well with others and didn't give god comments.The teacher often had to talk to me about my behavior
