Blood pressure & Pulse
	Poor 3 pts	Fair 4 pts	Good 5 pts
Explain procedure to client	Poor Forgot to ask if there are any problems and what the normal is, but did explain the procedure.	Fair Explained to client but forgot to ask what normal pressure is.	Good Explained to client what you will be doing and asked if there is any problem. Also asked what their normal blood pressure is.
Apply blood pressure cuff	Poor Needed several tries to do this and had to adjust the tightness.	Fair Did some fumbling with the cuff, but got it on correct.	Good Palpated the pulse point in the arm and Applied to left arm with the gauge in the front at the pulse point. Put it on at just the right tightness
Took blood pressure	Poor Needed several attempts to get the cuff inflated and left it up too long causing some pain.	Fair Did correct but took several attempts to get the inflation knob correct.	Good Handled the gauges corrected and inflated the cuff just above where it needed to be. Let the air out slowly but not painfully slow.
Obtained reading	Poor Obtained the correct reading on the second try but demonstrated little confidence.	Fair Obtained the correct reading on the second try.	Good Obtained the correct reading on the first try.
Pulse	Poor Had difficulty finding the pulse at the wrist, but fianlly was able to get this correct.	Fair Found the pulse at the writst after several minutes.	Good Found the pulse quickly at the wrist and took it correct the first attempt.
