DIAGNOSIS

4 = A

diagnosis is identified and is clearly supported by assessment data, and reflects accurate clinical judgement. The Dx is appropriate for patient, NANDA approved, and written in correct format.
3 = B

The diagnosis identified is appropriate for patient and is NANDA approved and clearly supported by assessment data. May not be listed in order of priority, or lacks the proper format.
2 = C

Diagnosis is not NANDA approved, appropriate for patient or not prioritized. Diagnosis not clearly supported by assessment data.
1 = F

Diagnosis portion is incomplete or irrelevant.
EXPECTED OUTCOME (Goal)

4 = A

The Goal identified is clearly related to the nursing diagnosis, are written in a patient focused manner, and are realistic. The goal contains clear criteria for measurement and a time frame for evaluation.
3 = B

The Goal statement is identified and relates to the nursing diagnosis, and is patient focused. Criteria for measurement included but may not be clear or contain realistic time frame for evaluation.
2 = C

The Goal statement is not relevant to the nursing diagnosis. The outcome is not client centered, measurable or has an achievable target date.
1 = F

Goal portion is incomplete or completely unrelated to nursing diagnoses.
INTERVENTIONS

4 = A

Identifies at least 5 or more specific interventions for the outcome criteria in order to help the patient reach the desired goal. Interventions are specific in action and frequency (how often).
Interventions demonstrate they were written with additional time & effort. All Data is measurable.
3 = B

Identifies 5 or more interventions for the goal that relate to the etiology of the nursing diagnosis. Interventions demonstrate they were written with moderate time and effort. Some Data is not measurable.
2 = C

Interventions demonstrate they were written with minimal time and effort. Interventions are unclear or do not clearly focus on the etiology of the nursing diagnosis or relate to patient goal outcome.
1 = F

Interventions demonstrate they were written poorly, with poor time & effort. OR the Interventions portion is incomplete, and/or are not relevant to patient.
RATIONALE

4 = A

Identifies at least 5 or more specific Rationales that appropriately correlate with each intervention. Each one required additional time and effort to create, were researched, and demonstrated utilization of critical thinking.
3 = B

Identifies 5 or more specific Rationales that appropriately correlate with each intervention. Each one required moderate time and effort to create, were moderately researched, and demonstrated a moderate utilization of critical thinking.
2 = C

Rationales provided do not demonstrate an understanding of the purpose of the interventions and/or are written with minimal effort, minimal research, and/or minimal critical thinking.
1 = F

Rationales provided do not demonstrate an understanding of the purpose of the interventions and/or are written with poor effort, poor amount of research, and/or poor critical thinking.
Evaluation

4 = A

Evaluation portion contains ALL the required information such as: The outcome data to prove the goal, and if the goal was met, partially met, or unmet.
3 = B

Missing ≤ 1 portion of the required evaluation statement but contains all the remaining required information such as: the outcome the data to prove the goal, and if the goal was met, partially met, or unmet.
2 = C

Missing ≤ 2 portions of the required evaluation statement but contains all the remaining required information such as: the outcome the data to prove the goal, and if the goal was met, partially met, or unmet.
1 = F

Evaluation portion is missing >2 portions of the required evaluation statement and/or is incomplete or does not relate to the diagnosis, goal statement or interventions.
