Medication Administration This is an evaluation to assess a learners ability to correctly administer oral medications

Identification of Client properly identifies client with two forms of identification	Poor learner does not confirm identity of client	Fair learner looks at name band but does not ask DOB and name	Good learner confirms name and DOB with pt and name band	Excellent Learner has confirmed correct patient and confirms name and DOB prior to preparing medications and once entering pt room to administer
Explains procedure to client verbally explains to client they will be administering medications and describes each medication client will be receiving	Poor Learner does not notify client of medication they will be taking	Fair Learner communicates to client they will be administering medications, yet learner does not explain each medication	Good Learner communicates to client they will be administering medications and states each medication they will be administering	Excellent Learner is able to communicate with client what medication they are administering and thoroughly describes each medication
