Poor

Requires adult to bring student to therapy at least 90% of the time. May come to therapy independently up to 10% of the time.
Fair

Requires adult to bring or prompt student to come to therapy at least 50% of the time. May come to therapy independently up to 50% of the time.
Good

Independently comes to therapy without prompts up to 75% of the time. May need prompts to attend at least 25% of the time

Comes to therapy at least 90% of the time. Student may require a prompt to attend on occasion, or may miss therapy due to a special event up to 10% of the time.
Participation in therapy

Poor

Student does not participate in therapy without high levels of encouragement, reinforcement and support to be engaged.
Fair

Student participates in therapy by responding but continues to need high levels of and consistent reinforcement.
Good

Student participates in therapy without difficulty using intermittent reinforcement.

Completes assignments

Poor

Student does not complete assignments given in or out of therapy.
Fair

Student attempts assignments and/or completes up to 50% of assignments .
