One Utilizes pictures, diagrams, etc. to illustrate treatment. Treatment plan is detailed but succient, and addresses the treatment goals. An evaluation plan is designed to determine if treatment should be terminated, should continue, or be modified.

Includes grammar and spelling.

Information scattered and disorganized. Grammar and spelling so poor, sections of paper are impossible to understand.
