Essential Tasks 33 pts 1. Student Introduces self. 2. Verifies patient information. 3. Identifies reason for visit. 4. Obtains vital signs. 5. Patient Education/Information regarding visit	Poor Student fails to perform 2 or more required tasks.	Fair Student fails to perform 3 or more required tasks.	Good Student fails to perform 4 or more required tasks.	Very Good Student performs all required tasks.
Progress Note/Charting 33 pts Progress Note should have following components: 1. Patient first & last name and DOB. 2. Chief complaint or reason for visit. 3. Date and timed. 4. Proper Grammar/Spelling 5. Signed (Medical Assisting Student) 5. Narrative Review of System.	Poor Students narrative and or written is inadequate with gross deficiencies.	Fair Student fails to address all elements in detail during presentation or has significant grammatical and spelling errors in case report.	Good Student addresses all key elements in narrative with minor grammatical errors, spelling, or formatting errors.	Very Good Student effectively correctly addresses every key element of the assessment in a narrative having the proper format with no grammatical or spelling errors.
Communication 33 pts Effective professional communication.	Poor Failure to communicate .	Fair Student does not explain assessment to patient or talks minimally to patient.	Good Student explains the assessment as they are completing each step.	Very Good Student explains the assessment as they are completing each step and is able to answer with confidence all patient questions in a calm and efficient manner.
