I completed my CW, HW and other assessments on time and accurately. Always wrote down ALL assignments. If I had questions I asked Mrs. Cheng in a timely fashion. I work at home on class material even when not asked to do so. I performed well on exams.

I completed my CW, HW and other assessments but may not have met all the criteria. I have hand in work late a few times and have forgotten to do a few assignments. I also forgot to write down assignments a few times. I did good on exams.

I did not complete my CW, HW and other assessments as directed. I often forget my homework and/or projects. Hardly ever write down assignments. I just barely passed or did poorly on exams.
Behavior/Effort

I was on task nearly all of the time.
I gave 100% in effort and tried my hardest.
I worked well with other students. Respect everyone in class.
Mrs. Cheng did not have to talk to me at all about my behavior. Ready to work when the late bell rings.

I was off-task occasionally
I put forth strong effort and worked fairly hard.
I worked well with other students.
The teacher had to talk to me about my behavior once. Almost always ready to work when the late bell rings.
Needs Massive Improvement

I was off-task a lot
I didn't try very hard.<
I didn't work well with other students.
Mrs. Cheng had to talk to me about my behavior more than once in this class. Disrespectful to classmate(s) or teacher. Always have to be told to concentrate and do the work.
