Behavior	Excellent The teacher did not have to talk to me at all about my behavior. I paid attention during class everyday.	Good The teacher had to talk to me about my behavior once. Effort/Attitutude	Excellent I was on task nearly all of the time. I gave 100% in effort and tried my hardest.	Good I was off-task occassionally. I put forth strong effort and worked fairly hard.	Fair off-task quite a bit I didn't try very hard.	Poor I was off-task frequently.	Conclusion How can you improve?
