Knocks on door, greet patient, introduces self, wash hands, check ID bracelet, provide privacy, lock wheels on bed, inform patient what your procedure will be conducted. Set up for procedure. Use proper body mechanics while performing assessment.

Performs less than two initial steps required in the skill performance.

Performs more than three initial steps required in the skill performance.

Performs all initial steps without difficulty in the skill performance.
FOCUSED ASSESSMENT

Asks the appropriate questions related to the assessment.
Using the appropriate techniques of:
a. Inspection
b. Palpation
c. Percussion
d. Auscultation

Fails to use at least one appropriate technique.
Fair

Uses less than two appropriate techniques.
Good

Uses less than three appropriate techniques.
Very Good

Uses all four appropriate techniques to perform the assessment and is able to answer all questions posed by patient and instructor.
REPORTING

2. History using initials and no identifying number
OBJECTIVE
1. Results of focused physicial examination.
3. Any follow-up action necessary.

Documentation is inadequate with deficiencies.

Fails to address all elements in detail or has significant grammatical and spelling errors
Good

Addresses all key elements in narrative with minor grammatical errors,spelling, or formatting errors.
Very Good

Effectively correctly addresses every key element of the assessment in a narrative having the proper formation with no grammatical or spelling errors.
COMMUNICATION

Effective professional communication orally & in writing.

Poor

Failure to communicate with patient during the assessment procedure.
Fair

Does not explain assessment to patient or talks minimally to the patient during the assessment procedure.
Good

Explains the assessment as they are completing each step.
Very Good

Explains the assessment as they are completing each step and is able to answer with confidence all patient questions in a calm and efficient manner.
FINAL STEPS

Inform patient that assessment is completed. Bed locked, bed in low position, all light within reach. Provide comfort measures and reposition patient in good body alignment. Wash hands before leaving room.

Poor

Does not perform any of the completion steps required at the end of the assessment process, i.e. bed locked, call bell within reach, disrespectful to the patient.
Fair

Fails to perform two or more of the necessary final steps to complete the assessment procedure.
Good

Fails to perform one or more of the necessary steps to complete the assessment procedure.
Very Good

Able to perform all the necessary steps to complete the assessment procedure.
