	Excellent 3 pts	Good 2 pts	Poor 1 pts
Daily Dress	Excellent - Always wears approved sneakers, change of clothes.	Good - Not dressed in proper attire 2-3 days	Poor - Not dressed in proper attire 4 or more days
Daily Participation	Excellent - Always participates in activities, tests, games, etc. - Displays superior "sportsmen-like" conduct	Good - Fails to participate in activities, tests, games, etc. on 2 or 3 occasions - Displays attitudes/actions that are not considered to be "sportsmen-like" on two or more occasions and - Parent contacted	Poor - Fails to participate in activities, tests, games, etc. on 4 or more occasions - Often displays attitudes/actions that are not considered to be "sportsmen-like" - Parent contacted - After school detention required
