Dress Code Free of odor, clean uniform, clean white leather shoes, trouser socks, hair pulled back, only neutral colored hair fasteners, no nail polish, no jewelry, name tag	Acceptable All components of clinical uniform in place in accordance with clinical dress code.	Average 1 component of clinical uniform missing.	Below Average 2 or more components of clinical uniform missing.	Unacceptable Frank violation of clinical dress code.
Demonstrate professional behavior Student displays positive attitude toward patients and staff. Communicates appropriately with staff and patients.	Acceptable All components of professional behavior are met.	Average 1 component missing.	Below Average Multiple components missing.	Unacceptable Unprofessional behavior, lack of communication skills, and/or poor attitude.
Rules and Safety Student follows all rules and regulations of the facility, observes safety measures according to OSHA guidelines and CDC recommendations, abides by confidentiality agreement with the facility.
