PREPARATION Enter criteria description	EXCELLENT Individual's portion of project was "ready to go" when it was time to combine all parts.	GOOD Individual's portion was "almost ready to go". We had to wait for this student to wrap up his/her part.	FAIR The student held up the final product but managed to get his/her part completed by the assigned due date.	POOR The student did not have his/her part completed and did not finish in time to add it in with the group. It was omitted or added in partial form.
COOPERATION Enter criteria description	EXCELLENT Always listens to, shares with, and supports the efforts of others. Tries to keep people working together.	GOOD Usually listens to, shares with, and supports the efforts of others. Does not cause problems in the group.	FAIR Often listens to, shares with, and supports the efforts of others.
