Sensory Testing
	Poor 1 pts	Fair 2 pts	Enter Title 3 pts
Explained purpose	Poor Explained, but not at the beginning and not as professional as it could be.	Fair Explained the test but not how they use this in life .	Enter Title Explained what you will be testing and how the client needs this in everyday life.
Explain test procedure	Poor Described the test, but struggled with this.	Fair Described the test but not as clear as possible.	Enter Title Described how the test will work. Showed on yourself if this is possible.
tested unaffected UE 1st	Poor Did this on the other extremity but only after coming back to it.	Fair Demonstrated on the UE but did this too long or too short to get an accurate understanding.	Enter Title Demonstrated how the test will work on the other UE and made sure the client has both the sensory ability and the ability to understand the test.
Occluded vision	Poor Asked the client to close their eyes.	Fair Occluded the client's vision but touched the client ocassionally and thought of it after test began.	Enter Title Made sure that you occluded the client's vision the entire time. On position sense it is appropriate to ask the client to close their eyes.
Performed test	Poor Performed the test correctly with no more than slight errors but did not appear confident.	Fair Perfomed the test with slight errors that did not affect the score.	Enter Title Performed the test accurately, toucing hard enough, wiping off the water tubes, not hurting the skin, etc.
Tested all dermatones	Poor Not organized and missed one area.	Fair Tested all dermatones but not in organzied manner.	Enter Title Turned arm over and tested all parts.
recorded accurately	Poor One slight error on the form as far as the shading goes.	Fair Did this correct but on the second try.	Enter Title Recorded on the form in the correct location, on the correct arm and with the correct symbol.
