Warm up 10 pts Demonstration & Initial Physical Activity	Poor Refused to stay within in assigned area and no attempt to try warm up activity.	Fair Stayed in assigned area but did not listen to instructions	Good Stayed in assigned area and participated for more than half the time.	Excellent Stayed in assigned area and participated to the best of ability.
Activity 10 pts Lesson	Poor Refused to participate or need constant teacher redirection to participate.	Fair Participation for less than half of the class.	Good Participated for more than half the class.	Excellent Participated the entire class and gave their best effort.
Followed Instructions 10 pts Makes an effort to do activity and aware of teacher expectations for the activity	Poor Refused to follow teachers instruction and/or disruptive to class after 3 prompts.	Fair Followed teacher instruction but had to have more than 2 prompts.	Good Followed teachers instructions with 1 to 2 prompts.	Excellent Followed teachers instructions the first time.
