Change for challenging behaviors 
This rubric scores the student on number of behavioral episodes, the intensity of behavioral episodes and the ability to self regulate during situations. It is scored so that the higher the percentage, the more frequent, intense the behavior and the less self-regulation used. Goal is to decrease the challenging behavior.

Exhibits the next behavior within the chain of the episode (e.g. Screams and stomps)
Fair

Exhibits the next behavior within the chain of the episodes (e.g. Property destruction such as throwing, pounding or banging objects)
Poor

Full episode and may include self-injury or injury of others
Self-regulation

Excellent

Calms self or self regulates almost daily
Good

Calms self or self regulates 4-5 times per week.
Improving

Calms self or self regulates 1-3 times per week.
Fair

Requires external cues to calm self or self regulate one time per day.
Poor

Requires external cues to calm self or self regulate more than 1 time per day.
