Student Performance Self-Assessment
	Excellent 4 pts	Good 3 pts	Fair 2 pts	Poor 1 pts
Classwork	Excellent All classwork was completed on time and accurately. All work was shown.	Good Classwork was completed on time and is mostly accurate OR is all accurate and mostly complete	Fair Very little classwork was done; or classwork was done with many errors	Poor No classwork was done.
Homework	Excellent All homework brought in on time, complete, accurate, and with all work shown	Good Homework was brought in on time at least 3 days a week. It was done accurately and work was shown	Fair 2 or more homework assignments missing or homework was turned in with many errors or significantly incomplete	Poor No homework was turned in.
Attendance/Preparedness	Excellent Brought all needed materials (including book, paper, and a pencil) to class every day Arrived to class on time every day	Good Materials brought to class 4 of the 5 school days. Arrived on time at least 4 of the 5 days.	Fair Forgot materials more than once, but not every day. Arrived at class on time most days.	Poor Without needed materials every day OR Arrived late frequently.
Behavior/Effort	Excellent Student was on task nearly all of the time. Student gave 100% in effort and tried his hardest. The teacher did not have to talk to the student at all about his behavior.	Good Student was off-task occassionally Student put forth strong effort and worked fairly hard. The teacher had to talk to me about his behavior once.	Fair Student was off-task quite a bit Student didn't try very hard. The teacher had to talk to the student about his behavior more than twice.	Poor Student was off-task frequently. The teacher had to talk to the student 3 or more times about his behavior.
