Preparation	Oustanding Always prepared and in proper PE apparel and footwear. On time and dressed.	Good Usually prepared and in proper PE apparel and footwear.	Fair Inconsistently prepared.	Poor Seldom prepared.	No Attempt Never prepared.
Participation/Effort	Oustanding Gives a great effort, models a high level of participation.	Good Gives a good effort, usually without prompting.	Fair Needs repeated prompting for participation.	Poor Uncooperative, and does not demonstrate the required skills.	No Attempt Refuses to stay on task.
Cooperation/Attitude/Behavior	Oustanding Models exemplary behavior and sportsmanship. Encourages and includes others; uses appropriate level of competition for the class. Shows respect for all classmates and teachers.	Good Supportive of all students, follows directions, respects equipment and shows respect.
