Number of questions	Poor Student includes 1-4 questions on survey	Fair Student includes 5-7 questions on survey	Good Student includes 8-10 questions on survey
Types and quality of questions	Poor Student uses only one type of question format (ex: multiple choice/one answer) and questions gather very basic data	Fair Students uses at least two types of question formats (ex: multiple choice and short answer) and questions gather opinions of respondents	Good Students use more than two types of question formats (ex: multiple choice/ one answer, multiple choice/multiple answer, essay, etc) and questions gather in-depth information from respondents
Number of responses	Poor Student gathers 1-3 responses	Fair Student gathers 3-6 responses	Good Student gets 7-10 or more responses
