Obtain and record past and present medical information useful in diagnosing, treating, and diagnosing the present condition of the eyes to include chief complaint, past ocular history of the eyes, family ocular history, review of systems, medicines, and allergies. Record the information completely, clearly, and accurately in the appropriate place, using accepted abbreviations only.

More then one component of the reading is inaccurate. More then one error in the documentation

Student is greater then + or - 0.25 in the sphere power or cylinder power or bifocal add or greater then 5 degrees away from the axis. Only one error was made in the documentation

Use a lensometer to perform the following: 1) Adjust the eyepiece 2) measure correctly, within +/- 0.25 diopters, both the spherical and cylindrical components of corrective lenses and within 5 degrees of axis. Calculate the bifocal add. Record the calculations completely, clearly, and accurately in the appropriate place, using accepted abbreviations only.

Check the pupil’s response to accommodation, direct light stimulus, consensual light stimulus, and swinging flashlight test. Reactions may be noted as brisk, sluggish, or none at all. Include the following for each: A) which eye the information is regarding. B) Intraocular pressure measurement, C) time of measurement, D) remarks on the patient’s cooperation.
