Basic Golf Swing Mechanics Assessment for the basic golf swing for 6-8 grade levels.
	Never The student demonstrates little or no proper form in all attempts. No aspects of UPF are visible. 1 pts	Sometimes The student demonstrates proper form for half or more of his/her attempts. At least one aspect of UPF is visible. 2 pts	Always The student demonstrates proper form on all attempts. All aspects of UPF are visible. 3 pts
Takeaway-UPF Left arm (for right handed students) is straight and shoulders turn to complete rotation.	Never	Sometimes	Always
