Care Plan and nursing process practice

Use the nursing process to research a health concern in a specific community. Explain the issue, provide statistical about the health issue, present a possible intervention, evaluate the intervention, and answer questions about the issue. The intervention will be evaluated for effectiveness and usability. Students are expected to collaborate effectively to create the finished product using reliable and up to date resources to create an intervention that is usable in a community setting

At Mid term and Final the Preceptor will evaluate the student Preceptee using this form.

Students are required to perform nursing skills with 100 % proficiency. In addition to skills performance of skills, students must use critical thinking skills to include indirect care measures:safety, infection control, comfort measures, resident rights.

Demonstrate the appropriate techniques for assessing a patient. Demonstrate the appropriate communication skills with patients while doing your assessment. Use of good body mechanic while performing a head-to -toe assessment. Assess the pertinent aspects of the patients condition.
