Student is able to provide 30 pts an explanation about accommodations and/or modifications needed	Poor Student does not know what accommodations and/or modifications he/she receives nor acknowledges reasons for them.	Fair Student states accommodations and/or modifications, but can not provide reasons for having them	Good Student is able to state accommodations and/or modifications with an explanation of why he/she is receiving them.
Student is able to communicate 40 pts needs or accommodations to appropriate person	Poor Student is unable to communicate needs to appropriate person.	Fair Student is able to state accommodations, but unable to when needed	Good Student is able to fully explain accommodations and needs to appropriate person.
